Consultation Rates
Clinic consultation rate is billed at $200/hr and to be charged at the end of any in-person consultation.
MVA patient consultation rates are individually assessed with each patient and negotiated with patient based on a variety of conditions and circumstances. Often no payment upfront is required for entitled insurance claimants who are off work, unemployed or on leave due to your MVA injuries. Payment will be deferred until your claim is settled.
Appointment Booking
1. Introduction These terms and conditions govern the booking of consultations with DAC Medical Rehab Admin Solutions Inc. By booking a consultation with us, you agree to be bound by these terms and conditions. Please read them carefully before proceeding with the booking.
2. Consultation Booking Process 2.1. Booking a consultation with DAC Medical Rehab Admin Solutions Inc. can be done through our official website or by contacting use by telephone or email. 2.2. The consultation will be scheduled based on the availability of our experts and your preferred time slot.
3. Consultation Fees 3.1. A consultation fee will be applicable for booking our services if your are a clinic, the amount of which is listed above. 3.2. The consultation fee must be paid in full at the upon completion of your consultation.
4. Cancellation and Rescheduling 4.1. If you wish to cancel or reschedule your consultation, you must provide a minimum of 48 hours’ notice before the scheduled appointment time. 4.2. Cancellations or rescheduling requests made less than 48 hours before the scheduled appointment will be subject to a cancellation fee. 4.3. The cancellation fee will be equivalent to 30% of the consultation fee. 4.4. Cancellation requests can be made through the scheduling email that was sent at the time of booking or by contacting us by email or phone.
5. Non-Attendance 6.1. If you fail to attend the consultation without providing any prior notice, a cancellation fee with apply and non-attendance without notice may also affect your eligibility for rescheduling future appointments.
6. Expert Availability 7.1. While DAC Medical Rehab Admin Solutions Inc. will make every effort to ensure the availability of our experts, circumstances beyond our control may sometimes require us to reschedule or cancel appointments. In such cases, we will provide you with the option of rescheduling or receiving a full refund.
7. Confidentiality 8.1. All information shared during the consultation will be treated with the utmost confidentiality.
8. Liability 9.1. DAC Medical Rehab Admin Solutions Inc. will not be liable for any losses, damages, or inconvenience resulting from circumstances beyond our control, including technical issues, natural disasters, or other unforeseen events.
9. Modification of Terms 10.1. DAC Medical Rehab Admin Solutions Inc. reserves the right to modify these terms and conditions at any time without prior notice. The updated terms will be effective for all future consultations.
